Do you need a residential facility?

Buying a residential facility

Many home adaptations or home aids can be bought in shops. You cannot obtain these through the local authority. For example: a wall bracket, doorstep aid, higher toilet seat, shower chair, non-slip bath mat, bath shelf or a stand-up chair.

Moving to a suitable home

Often, moving to a ground-floor home or one built specifically for elderly or disabled people is a good solution.

Borrowing money to adapt your home yourself

With a home equity loan you can adapt your home to live in it longer, safely and enjoyable or to make it more sustainable. Examples include an adaptation in the bathroom, a bedroom on the ground floor or adaptations for access to the house or garden. More information can be found on the Home equity loan page.

Support under the Long-Term Care Act

Do you need day and night care or supervision because of an illness or condition?
If so, you may be eligible for care at your home or a stay in an institution under the Long-Term Care Act (Wlz). Apply for Wlz care at the CIZ (Care Needs Assessment Centre).

A residential facility under the Social Support Act (Wmo)

Are there no other solutions for you? In that case you may be eligible for residential facilities under the Wmo. For example, a stair lift, ceiling lift or hoist. Or an adaptation of the toilet, shower or kitchen. Contact the City of Leeuwarden to discuss this.

How do I submit an application?

Submit your application online (you will need DigiD). Are you unwilling or unable to apply online? Download and complete the Wmo Application Form. Send the form by post to the City of Leeuwarden. You can also hand it in at the Town Hall.

What happens next?

A local authority employee will call you within 6 weeks of receiving your application. The employee will first ask for more information about your situation. This can be done through a telephone interview or a meeting at your home. A solution is sought together with you. The local authority will let you know within 2 weeks of the interview whether you are eligible for a residential facility. You pay a personal contribution for a facility under the Wmo. If there are many applications at once, it may take a little longer. The local authority will inform you of this.
