Health certificate

For example, if you are 75 or above, have a disability or want to apply for an HGV or bus driving licence, you have to complete a Health Certificate. This contains questions about your health. The fastest and cheapest way to arrange this is online (using DigiD) via the website of the Central Office for Motor Vehicle Driver Testing (CBR).

Are you unwilling or unable to apply online

In that case, you have the option of collecting a paper health certificate from the Town Hall. You can only collect a health certificate for 75 years or older. Any other applications for health certificates must be sent to the Central Office for Driving Skills Certificates (CBR).

What happens when you have submitted an application

The Central Office for Driving Skills Certificates (CBR) decides whether you are healthy enough to drive. You will be informed of this automatically.
